Radiograph

Ankle internal/external oblique view

Japanese ver.
RadTechOnDuty (medial oblique)

Purpose
Internal rotation : It provides excellent visualization of the tibiofibular joint, the lateral aspect of the talocrural joint, and the base of the 5th metatarsal.
External rotation : It provides excellent visualization of the medial aspect of the tibiofibular joint and the anterior and posterior tibial tuberosities.

Prior confirmation
Remove any obstacles.
Do not force dorsiflexion if there is pain.

Supine position.
Align the lower leg axis of the examined side with the cassette’s long axis.
Align the center of the medial malleolus and lateral malleolus with the cassette’s center.
Place the RL marker.
Internal rotation :
Place a positioning block under the knee of the examined side.
Adjust the height of the cassette so that the lower leg axis becomes parallel to it.
Slightly dorsiflex the ankle joint on the examined side to prevent the calcaneus from overlapping the lateral malleolus.
Internally rotate the knee joint to 35° or 45° to ensure that there is no torsion from the knee joint to the ankle joint.
-> The 35° internal rotation position provides excellent visualization of the tibiofibular joint.
External rotation :
Externally rotate the knee joint to 45° to ensure that there is no torsion from the knee joint to the ankle joint.

CR, distance, field size
CR : Perpendicular incidence towards the cassette center (the midpoint between the medial malleolus and lateral malleolus).
Distance : 100cm
Field size : Including the lower third of the lower leg to the base of the fifth metatarsal.

Exposure condition
50kV / 4mAs
Grid ( – )

Clear visualization of the cortex and bone trabeculae.
Inclusion of the base of the fifth metatarsal.
No overlap of the non-examined lower limb.
Tolerance for soft tissue visibility.
Presence of the RL marker.
Absence of motion-induced blur.
Internal rotation :
Clear visualization of the tibiofibular joint.
Minimal overlap of the fibula with the tibia.
No overlapping of the calcaneus with the lateral malleolus.
External rotation :
Clear visualization of the medial aspect of the tibiofibular joint.
The fibula projects to overlap with the tibia.
